The Housing Navigator is responsible for engaging families and individuals experiencing homelessness and supporting them to ensure that they are successfully connected to a permanent housing opportunity. Housing Navigators use an assertive engagement approach to maintain contact with families and individuals and to support them throughout the housing process. Housing Navigators are mobile and a car is required to conduct field visits throughout the city of Chicago. This is a full-time position with some evenings and weekends required.
Facing Forward is expanding its team! The Case Manager is responsible for providing comprehensive case management to individuals and families residing in project-based permanent supportive housing (PSH) using the service models of harm reduction, housing first, trauma-informed care, and two generation support. Responsibilities include supporting clients directly via an individualized service plan, on-going monitoring, and evaluation. This is a full-time position with some evening and weekends as required.
The Scattered Site Case Manager is responsible for providing home-based case management for families and individuals formerly experiencing homelessness with disabilities (including mental illness and substance use disorders). Case Managers support residents in maintaining housing, increasing or maintaining income, and gaining life skills and provide services to all members of the household, including children and youth. A car is required to conduct home visits throughout the city of Chicago. This is a full-time position with some evenings and weekends required.
